For the ending, around the 3 min mark, start the sped-up middle part again, the A and D  
notes on the B and E strings, and then the D chord. 

And around the 3:23 mark, a second guitar playing D and C# notes at a higher octave  
comes in. You incorporate this by playing the D chord like this
